[kernel] Remove last of old allocator interface

Removing the `allocator.h` file defining the `kutil::allocator`
interface, now that explicit allocators are not being passed around.
Also removed the unused `frame_allocator::raw_allocator` class and
`kutil::invalid_allocator` object.
